Medical Record Request: Printed Copies
To request printed copies of your medical records from Anaheim Community Hospital, please follow these steps:
- Download and print our Release of Information form – Anaheim Community Hospital
- Complete, sign and date the form. In order to verify your identification and validate your authorization, you are required to include a legible copy of a valid photo identification (i.e.: a driver’s license, a military ID or a state ID).
- Email, fax, mail or deliver your paperwork in person. Please see further instructions below.

Please allow up to (15) days for processing from the date of receipt of the completed authorization.
Copy service/subpoena requests must be hand-delivered to personnel in our Health Information Management Department (with the exception of criminal subpoenas; via certified mail or hand-service accepted). Fees for services will be billed by a clerical hourly rate and $0.25 per page.

Urgent Requests/Records for Your Physician
For immediate continuity of care requests, you or your healthcare providers may request that records be sent directly to their office.
The physician’s office must send a written request on a business letterhead to 714-699-5591 (fax). Please indicate the patient’s name, date of birth and date of visit/admission. Please indicate “STAT” at the top of the request. For immediate assistance, call 714-243-9001.
